Pills in the movies

Real life does sometimes appear in movies. We tell you this, not to promote self-treatment, but to introduce the idea of acupuncture. Here specialists take a set of long needles and then drive them into your skin. Unfortunately, those of you who like horror movies will be disappointed at the outcome. Instead of rivers of blood erupting from gaping wounds like the oil from a gusher, nothing appears to happen. The victims lie calmly watching the needles go in, all the while chatting about the latest fashions or the current World Series with the expert. It's all very disappointing except, if you talk to any of the millions of people in China or South East Asia where acupuncture is a routine part of medicine, they all report excellent results from the treatment.

Indeed, in China, acupuncture is used alongside conventional anesthetic. Patients going through surgery remain conscious and discuss the finer points of the surgeon's technique with the theater nurses. To those of us in the West, this is taking things too far. The very idea of it really does seem like the plot from a horror movie. So we are not asking you to believe in anything so extreme. Rather we come down a few notches in ambition and report the latest results of a clinical trial in Seattle. A team of dedicated scientists gathered a group of 650 people, all of whom had serious lower back pain with loss of mobility. They were divided into four groups. One received genuine acupuncture from an expert. The second was pricked by toothpicks in all the right places. The third was pricked with needles at random (faintly disconcerting). While the final group was given conventional physical therapy.

In follow-up interviews over the next twelve months, the participants were asked about the level of pain and whether there were any improvements in mobility. Less than half those receiving physical therapy reported any significant change in their condition. But 60% of those receiving the "acupuncture" treatments reported significant and lasting improvements. So the results show a very strong placebo effect for both real and simulated acupuncture. The participants were prepared to believe they were going to find relief, and that's what they got. This strongly suggests the mind is a key player when it comes to lower back pain and, if people believe in the treatment, they get up to twelve months of benefit. New evidence supporting acupuncture

For centuries, Traditional Chinese Medicine (TCM) has been using acupuncture to treat a wide range of different diseases and disorders. Put most simply, millions of people accept treatment based on sticking needles into their bodies. This sounds a strange thing to do. Western doctors have been consistently skeptical yet, when you actually look at the theory behind it, there is a remarkable similarity in approach. TCM relies on mapping the human body to identify the critical points for manipulation. These points are essentially the same as the major groupings of nerves recognized as significant in the West. Pain-blocking injections are made into TCM acupuncture points. With reluctance, Western researchers are looking more closely for evidence whether acupuncture is effective. Reluctance because the major drug manufacturers discourage research that might put them out of business. Recently, a clinical trial in Seattle was written up in the Archives of Internal Medicine. Some 650 people suffering back pain were divided into four groups. One received genuine acupuncture, the second a modified version of acupuncture pricking the skin in the right places with wooden toothpicks. The third group were pricked with needles at random. The fourth received conventional physical therapy.

All the participants were given ten sessions over a seven-week period. There were follow-up interview spread over the next year to establish whether there was any lasting effect from any of the treatments. Interestingly, 60% of those receiving the different versions of acupuncture were still reporting benefits at the end of the year. Only 40% of those receiving physical therapy reported any continuing improvement. At one level, this would suggest there is a strong placebo effect from the use of acupuncture, i.e. those who receive the treatment believe it will be effective and so it proves to be. In scientific terms, there's nothing wrong with this outcome. When 60% of participants report a better quality of life twelve months after the treatment ended, this is strong evidence of the effectiveness of the treatment. Physical therapy is also based on social contact with one human selling the effectiveness to another yet this got a smaller placebo effect.

It's notoriously difficult to diagnose the cause of back pain. This suggests doctors should be very open-minded when looking for possible treatments. When the mind and body are so closely involved, engaging the mind with acupuncture, no matter whether real or simulated, seems more effective than conventional Western therapy. This is not to deny the role of a painkiller like Tramadol. When the pain grows more serious and mobility is affected, everyone will benefit from taking the drug. But this resort to a drug should be seen as part of a general strategy to encourage mobility. If this is your back, you can take the Tramadol for a short period of time. This should get you through the program of physical exercise, stretching, and hot and cold therapies designed to reduce any swelling and develop better muscle tone to support painless movement. If your problem of back pain is persistent, you should talk to your doctor about acupuncture and some of the alternative therapies. You may find more relief from these treatments than you expect.

Can you live with your baldness?

This has been justified by the inevitable "truth" that "all patients are different". So, within preset limits, doctors play with different drugs in combination and at different dosages until the best outcome is achieved.

In two trials titled Circadian Administration of Prednisone in Rheumatoid Arthritis (CAPRA), EULAR has been testing a modified-release version of Prednisone. The idea is to take a low dose before going to bed. The tablet is designed not to release the drug until about four hours into sleep with the peak concentration being achieved about two hours later. So,someone going to bed at 10 p.m. will get the maximum effect of the drug at about 4 a.m. which is judged the ideal time if pain and stiffness on waking is to be reduced. The results from the second trial have just been released and confirm the success of the modified-release system. The drug was administered over a twelve week period, with a major short-term improvement in mobility on waking. But what is really significant is the continuing improvement in mobility when participants were allowed to continue taking Prednisone for up to twelve months after the end of the formal trial. It seems clear that the use of this drug alongside the optimized cocktail of DMARDs produces the best overall outcome with morning stiffness reduced and general mobility improved during the day. Given there is no immediate prospect of any drug coming on to the market as a cure for Rheumatoid Arthritis, these trials old out hope for those who currently struggle with pain in the mornings.

DON'T PANIC!

The two immortal words, always written in capitals, that appeared on the front cover of the Hitchhiker's Guide to the Galaxy. And just because it's been picked on for fame by science fiction, doesn't make it any less true for us stuck in the real world. In fact, when you count up the number of TV shows and movies where characters have run around in circles screaming, "Don't panic", it amounts to a supposedly comic epidemic. Not that it doesn't happen in the real world. But, when it does, we spectators always look away in embarrassment, sometimes tinged with fear. After all, this one may have a good reason to panic. Have aliens just landed? Is this food poisoned? It's easy to be paranoid. Panic is easily communicated. Try shouting "fire" in a crowded theater and find yourself arrested for some powerful criminal offenses when folk injure themselves trying to save themselves.

So how does panic get "disordered"? There's a moment when uncertainty starts to get out of control, when fear of what may happen begins to build. Most of us control this all-too-human apprehension. But, when the uncertainty and fear never fade, they can bring life to a halt. It's the paralysis we describe as the deer caught in the headlights. Except, people don't stay paralyzed as the truck bears down on them. This is when the deer becomes a headless chicken running around the barnyard.

People with panic disorders are made to feel ashamed by our reaction to them. At a time when they need our help and support, we are turning away. Because they grow to fear more panic attacks, it's easy to convert early symptoms into the full attack just by worrying about it. The disaster is provoked by a fear of that disaster. It most usually starts in the teen years and, unfortunately, more women than men have it - something which adds more shame to the men who cannot avoid displaying it.
